Your Appointment
Please come prepared to share information about your medical and surgical history. We also ask that you bring the following items:
- A list of current prescription and over-the-counter medications, including the dose and frequency
- Your insurance card
- Physician referral form if required by your insurance provider
- Pertinent information about your medical and surgical history
- History questionnaires (if mailed to you prior to the initial visit)
Payment and Insurance Information
Indiana Neuroscience Associates is a provider for Medicare and most major insurance plans. Insurance billing is provided; anything not covered by insurance will be responsibility of the patient or guardian.
Payment is requested at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work with patients who require a payment plan. Questions regarding your coverage and benefits should be directed to your employer or insurance company.
